Delivery Checklist
Pre-Register
To make your birthing experience as pleasant as possible, please pre-register for your delivery at Northwest Health - Porter. Even if you pre-register, it is still necessary to check in at the registration desk with each visit. You may print the registration form and return it by mail.
If you are scheduled for a cesarean section, you will receive a call from one of our charge nurse to obtain additional information so we can have the paperwork ready when you are preparing to deliver your baby.
To expedite the process even further, you may call (219) 983-8343 and ask to speak with the pre-admission nurse to schedule an interview and to learn more about your cesarean section and preparing for your new baby. This information will be used to help prepare your medical chart when you arrive.
What to Bring
While you are preparing for the birth of your child, the following list of items should help you plan what to bring when you come to the hospital:
- Your driver’s license and insurance cards
- A cell phone and charger
- A current list of medications
- Any items that you use on a regular basis, such as eyeglasses and/or contact lenses, comb, toothbrush and toothpaste, deodorant and shampoo
- A copy of your advance directive, living will and/or medical power of attorney – if you have one
- Comfortable clothing to wear home and a robe/slippers for your stay
- Infant car seat for the day of discharge
- An outfit for your baby to have pictures taken in and to wear home
Please leave valuables such as jewelry, credit cards and checkbooks at home.
