Professional Practice Model
Northwest Health - Porter nurses serve as the primary link between their patients, families, physicians and the rest of the organization. They are expected to exercise autonomy and authority consistent with the standards of nursing practice and professional ethics and apply abilities, skills and resources to achieve the mission of Northwest Health - Porter. Nurses are the central pillar of our patient care.
Nurses as Clinicians
Northwest Health - Porter nurses are expected to demonstrate clinical knowledge and technical skills and to utilize the nursing process in the provision of care across the continuum. The nurse is the bridge that connects the physician’s treatment plan to the patient’s goals while coordinating the work of the interdisciplinary care team and keeping the unique needs of the patient/family at the center of the care plan.
Nurses as Advocates
Northwest Health - Porter nurses serve as advocates working on behalf of others and representing the concerns of the patient, family and community. They have a particular interest in viewing each patient as an individual and thus recognize that care plans need to be customized to the unique needs of each patient.
